Please submit an Application Form for each Course (or service). Full details should be on the first Application Form but only the essential details (*) are required for subsequent Applications.

If necessary use "Additional Notes" at the bottom of the Form for clarification . Thank you.

After submitting this Form you will be asked for a NZ$500 deposit to secure your bookings. This may be made by:
